Diploma Pharmacist (B Grade)
Job Description
Accurate Dispensing - Review, verify, and dispense inpatient and outpatient prescriptions, ensuring the correct drug, dosage, route, and timing.
Clinical Screening - Screen physician orders for appropriateness, drug interactions, contraindications, and specialized dosing (e.g., renal/hepatic adjustments and high-alert medications).
Critical Care Support - Facilitate the rapid and accurate supply of emergency and life-saving medications, particularly during night shifts or in ICU scenarios.
Controlled Substances: Enforce strict compliance regarding the handling, storage, and documentation of narcotics and psychotropic drugs.
Stock & Storage Control: Maintain optimal shift-level inventory and ensure compliance with proper storage conditions (e.g., cold chain, high-alert isolation).
Quality Safeguards: Conduct proactive expiry tracking and manage the replenishment of crash carts and emergency boxes.
Shift Transitions: Deliver comprehensive shift handovers detailing pending tasks, critical patient alerts, and stock discrepancies.
Interdisciplinary Liaison: Serve as the point of contact for nursing and medical staff, providing accurate information on drug availability, dosing, and administration.
Regulatory Alignment: Ensure all operations adhere to hospital policies and national pharmaceutical laws.
Risk Management: Document and report medication errors, near-misses, and adverse drug reactions (ADRs).
Team Leadership: Supervise and review the work of pharmacy assistants/technicians to maintain accuracy and efficiency.
Execute additional tasks assigned by management.
Experience
